CHRISTUS Health believes that the impact of a successful CHNA must extend far beyond its regulatory necessity. Having just completed their latest CHNA, they wanted the document and its data to guide the health equity strategy for all areas of the health system.

Leaders began by expanding upon the traditional data-gathering process to create a community feedback loop via data tools and a new community survey. Their goal was not only to identify and understand the health needs of communities but also to ensure that the CHNA and its supporting work would be easy to use and transparent in presentation – making it more actionable for everyone.

In addition to identifying the major health needs in communities, CHRISTUS Health wanted to pursue more precision with their community work in order to reach those most in need. To do this, they developed a process using a combination of datasets to identify communities of focus.
